CareChronicle is leveraging advances in AI to reduce the negative outcomes associated with poor patient health literacy. Approximately 54% of all Americans do not possess the requisite health literacy to navigate our health system, and the resulting costs and damages are as much as 17% of ALL healthcare expenditures ($612B/annually). It is well documented that poor health literacy drives patient morbidity & mortality, increases preventable ED visits, increases 30-day readmissions, and increases non-billable provider time spent messaging with patients. Hospitals attempt to address these problems, but current solutions are manual and dependent on already scarce personnel.
